Instructions for Submission

Please review the Entry Requirements and Entry Check List before submitting your entry.

The following award nomination form is an Adobe PDF file. The file contains interactive fields so that you can type directly into the document and send your submission via e-mail as an attachment (to awards@nfba.org). This file will work with any version of Adobe Acrobat—you do not need to download a new version.

Please be sure to click “Save a Copy” at the top of the PDF file once you open it. Save the file to your computer so that you can access it again if you need to make changes and if you would like to keep it for reference.

To type in the file, simply position your cursor where you would like to type—move your mouse to the line and click it. To move to a new line, use your mouse to click the line and begin typing again.
